Signs
Gonorrhoea nosode.
NEISSERIA The genus Neisseria contains a number of species which are normal flora and pathogens of humans and animals; of these, the pathogenic species N. gonorrhoeae and N. meningitidis are of human origin. Strains of N. meningitidis may be carried as normal flora in the throat. Under some circumstances, N. meningitidis may also be pathogenic, causing sporadic cases or epidemics of meningitis. Between 3% and 30% of healthy persons in nonepidemic geographic areas may be asymptomatic carriers of N. meningitidis i.e. , meningococci have colonized their throats without causing disease. The carrier state may persist for many months. N. meningitidis causes epidemic meningitis in many parts of the world such as Sub-Saharan Africa. Certain types of N. meningitidis are usually associated with meningitis. Other Neisseria species are considered to be commensals i.e. , they colonize the host without causing disease. Strains of these species are normal flora in the throat. 1
GONORRHOEA Gonorrhoea is an ancient disease. An Assyrian tablet speaks of thick or cloudy urine, and the Hippocratic writers refer to ‘strangury’. It is caused by a bacterium called Neisseria gonorrhoeae, named after the German bacteriologist Albert Neisser [1855-1916] who in 1876 identified the gonococcus causing gonorrhoea. The bacterium grows and multiplies quickly in moist, warm areas of the body. Infections are most commonly found in the urethra in males and the cervix in females, and are transmitted by sexual contact. The throat, rectum and eyes of both sexes can also be affected. Gonococcal infections are 1.5 times as common in men than in women, although serious sequelae are much more common in women. Gonorrhoea is a significant health problem worldwide, with approximately 200 million new cases of gonorrhoea appearing each year. In the United States 133 cases occurred per 100,000 people in 1998, showing an increase of 11 cases per 100,000 compared to 1997, while the rate had fallen 64.2% from 1985 to 1997. Due to the dramatic rise of antibiotic-resistant strains of the gonococcus, the treatment of gonorrhoea is also an increasing problem.
SYMPTOMS The onset of symptoms is from 1 to 30 days following infection, with 3-5 days as an average. Common signs are discharge of pus from the urethra and mild to severe burning pain when urinating. Females will have an unusual vaginal discharge, or pain on urination. Up of 20% of infected men will not have any symptoms, whilst 80% of infected women may be without symptoms. More advanced problems, indicating progression to pelvic inflammatory disease [PID], include nausea, vomiting, ovaritis, lower abdominal pain, lower back pain, cramps during and between the menstrual cycle, vaginal discharge, fever, and pain or bleeding associated with intercourse. Internal scarring and blockage of fallopian tubes can result in sterility, tubal pregnancies, chronic pelvic pain and menstrual irregularities. Infected women can pass gonorrhoea to their newborn infants during delivery, causing eye infections in their babies. Blindness from neonatal gonococcal infection is a serious problem in developing countries but is now uncommon in the US and other countries where neonatal prophylaxis [with silver nitrate or erythromycin] is routine. In males, gonorrhoea can cause pain and swelling in the testicles, possibly resulting in sterility. The most common long-term sequelae of gonorrhoea are chronic pelvic pain in women after PID, septic abortion, chorioamnionitis in pregnancy, blindness after neonatal or adult conjunctivitis, and infertility of either sex.
DGI Disseminated gonococcal infection [DGI] occurs following approximately 1% of genital infections. It is seen more frequently in women, especially during menstruation and pregnancy. Patients with DGI may present with symptoms of rash, fever, arthralgias, migratory polyarthritis, septic arthritis, endocarditis, or meningitis. Joint or tendon pain is the most common presenting complaint. About 25% of patients with DGI complain of pain in a single joint, while up to 2/3 describe polyarthralgia, which often is migratory. Severe pain, swelling, and decreased mobility in a single joint suggest a purulent arthritis with effusion. The knee is the most common site of purulent gonococcal arthritis. Tenosynovitis also is common, usually affecting the small joints of the hands. Skin rash is a presenting complaint in approximately 25% of patients, but a careful examination will reveal a rash in the majority of patients with DGI, including maculopapular, pustular, necrotic, or vesicular rash, typically occurring on the torso, limbs, palms, and soles. The rash usually spares the face, scalp, and mouth. Haemorrhagic lesions, erythema nodosum, urticaria, and erythema multiforme occur less frequently. Headache, neck pain and stiffness, fever, and decreased sensorium may indicate gonococcal meningitis. This disease may be clinically indistinguishable from meningococcal meningitis on presentation, although the course of gonococcal meningitis usually is less rapid than that of meningococcal meningitis. Gonococcal endocarditis is more common in men than women. Patients with collagen vascular disease [especially those with systemic lupus erythematosus] also may be more prone to this complication. DGI can cause abscess formation within the soft tissues, presenting as localized tenderness, oedema, and pain with motion. 2
CHLAMYDIA Gonorrhoea often occurs together with chlamydia, another common sexually transmitted disease with an incidence, in the United States, of an estimated 4 million new cases each year. Symptoms usually appear within 1 to 3 weeks after exposure, if any symptoms occur at all; 50% of infected women and 25% of infected men develop no symptoms whatsoever. The symptoms of gonorrhoea and chlamydia are so similar that the only definitive way to diagnose chlamydial infection is attempting to grow chlamydia bacteria from a sample of secretions in specialized tissue culture in the laboratory. Up to 40% of women with chlamydia will develop pelvic inflammatory disease. Chlamydia is caused by the bacterium Chlamydia trachomatis. Pregnant women may pass the infection to their newborn infants during delivery, with subsequent neonatal conjunctivitis or pneumonia. Infant chlamydia pneumonia in the first month produces rapid, closely spaced staccato coughing with brief inspirations resembling whooping cough but without whoops or fever. In males chlamydia typically causes urethral infection with painful urination, but may also result in complications such as swollen and tender testicles.

The gonorrhoeal cocci a nosode
Introduction
Having been derived from gonorrhoeal cocci, it has been found most efficacious in eradicating any and every complaint, by whatever name they are called, brought on and consequent upon gonorrhoea. Medorrhinum bears the same relation to deep seated sycotic chronic affections of spinal cord and sympathetic nervous system as psorinum does to deep seated chronic affections of skin and mucous membrane and syphilinum to any syphilitic affection.

Causations 1. Maltreated and suppressed gonorrhoea (Arg. m., Arg. n., Nit. a., Sep., Staph., Thuja.). 2. History of gout, rheumatism, neuralgia and diseases of the spinal cord and its membrane, ending in paralysis which can be traced to sycotic origin. 3. Schirrhous carcinoma or cancer when symptoms point to history of sycosis. 4. In case of women, chronic ovaritis, salpingitis, pelvic cellulitis, fibroid, cyst or any other morbid growth with or without sycotic origin.

Constitution 1. Appearance : Medorrhinim is ideally suited to the patients having constitutional effects of maltreated or suppressed gonorrhoea, with trembling all over, weak, fatigued and emaciated as a result of long continued chronic disease, with a state of collapse. Ricketic children with pale appearance, dwarf and stunted growth mentally and physically (Bar. c.). 2. Temperament : Weeping disposition, peevish and impatient. 3. Relation with heat and cold : Medorrhinum is a hot patient, easily affected by heat in general. 4. Miasm : Sycosis is in the background for most of the troubles. 5. Diathesis : Rheumatic and gouty diathesis.

Mental generals 1. Great hurry in doing work gets fatigued (Arg. n., Syp., Lach., Puls., Cal. p.). 2. Time moves too slowly (Alum., Arg. nit., Can. ind.). 3. Forgets, what she had read, even that of the previous night 4. Forgets names, initial letter etc., has to ask even the name of his most intimate friend. 5. Loses the thread of her talk constantly (Lach.). 6. Sees large number of people, or rats running around in the room. 7. Sensation as if all life was unreal, like a dream. 8. Cannot speak without crying (Puls., Lil. tig.). 9. Fear of darkness (Can. i., Stram.). 10. Very impatient. 11. Irritated at tifles. 12. Selfishness. 13. Melancholy with suicidal thoughts. 14. Mental symptoms are aggravated when thinking about them. (Cal. ph., Helon., Oxa. acid.). 15. Anticipates incidents even before they occur.

Physical generals 1. Reaction to heat and cold : Medorrhinum is a hot patient, easily affected by heat in general. 2. Desires : Alcoholic drinks (Ars., Asar., Lach., Nux. vom.), green fruit, ice (Verat.), organges, salt things (Arg. nit., Carb. veg., Lac. c., Nat. m.), sour, acids etc. Sweets (Arg. nit., Chin., Lyc., Sulph.). 3. Sleep : Knee elbow position, lies on abdomen, back with hand on head. Wakeful, sleeps towards morning. Great restlessness at night, sleepy but can not sleep. 4. Sweat : Cold sweat during collapsed state. 5. Dream : Of the dead (Ars., Mag. carb., Thuja.), drinking, frightful, ghosts, spectres (Arg. nit., Carb. veg., Graph.) of walking. 6. Thirst : Constant thirst, even in dreams she is drinking. 7. Appetite : Ravenous hunger immediately after eating (Cina., Lyc., Psor.). 8. General modalities : (a) Aggravations : When thinking of it (Helon., Ox. ac.), heat, covering, stretching, thunderstorm, least movement, sweets, from day light to sunset (Rev. of syph.). (b) Ameliorations : At the seashore rev. of Nat. mur., lying on stomach, damp weather (Caust., Nux.).

Guiding symptoms 1. Great heat and soreness with enlargement of lymphatic glands all over the body. 2. Trembling all over (subjective), intense nervousness and profound exhaustion. 3. Pains arthritic, rheumatic, a sequal of suppressed gonorr-hoea, constricting, seems to tighten the whole body (Cac.), sore all over as if bruised (Arn., Eup.). 4. State of collapse, wants to be fanned all the time (Carbo. veg.), craves fresh air, skin cold, yet throws of the covers (Camph., Sec.), cold and bathed with cold perspiration (Ver.). 5. Burning of hands and feet, wants them uncovered and fanned (Lach., Sulph.). 6. Head : Intense burning pain in brain agg. in cerebellum, extends down the spine. Head feels heavy and is drawn backwards. Sensation of tightness and contraction, extends down the whole length of the spine. Headache and diarrhoea from jarring of cars. 7. Throat : Sensation as if she had taken a severe cold, with distressing aching in bones. Throat sore, and swollen, deglutition of either liquids or solids impossible. (Merc.). Throat constantly filled with thick, gray or bloody mucus flowing from the posterior nares (Hyd.). 8. Bowels : Stool tenacious, clay like, sluggish, cannot strain from a sensation of prolapse of rectum. (Alum.). Constriction and inertia of bowels with ball like stools (Lach.). Can only pass stool by leaning very far buckwards, very painful as if there was a lump in the sphincter, so painful as to cause tears. Sharp, needle like pains in rectum. Oozing of moisture from anus, foetid odour of fish brine (Caust., Hep.). 9. Urinary organs : Severe pain (backache) in renal region amel. by profuse urination (Lyc.). Renal colic, intense pain in ureters with sensation of passage of calculus. (Berb., Lyc., Ocim.), craving for ice. 10. Nocturnal Enuresis : Passes enormous quantities of ammoniacal high coloured urine in bed every night, agg. by overwork or over play, extremes of heat or cold, when the best selected remedy fails, with a history of sycosis. Painful tenesmus of bladder and bowels when urinating. 11. Sexual organs : Menses profuse, very dark, clotted, stains difficult to wash out (Mag. carb.). Metrorrhagia : At climacteric, profuse, for weeks, flow dark, clotted, offensive, in gushes, on moving; with malignant disease of the uterus. Intense menstrual colic, with drawing up of knees and terrible bearing down, labour like pains, must press feet against support, as in labour. Intense pruritus of labia and vagina agg. by thinking about it. Breasts and nipples sore and sensitive to touch. Breasts cold as ice to touch, especially the nipples, rest of the body warm (during menses). 12. Respiratory Organs : Asthma : Choking caused by spasm of epiglottis, larynx closed, so that no air could enter, only amel. by lying on face and protruding the tongue. Soreness of larynx as if ulcerated. Dyspnoea and sense of constriction, can inhale with ease, but no power to exhale. (Samb.). Cough : dry, incessant, severe, painful, as if mucous membrane was torn from larynx, deep, hollow, sensation as if coughing into a barrel agg. at night, from sweets, on lying down amel. by lying on stomach. Sputa : albuminous, frothy, small, green, bitter balls, viscid, difficult to raise. Incipient consumption, severe pains in middle lobes of the lungs. 13. Back and Extremities : Pain in back between scapulae, whole length of spine sore to touch (Chin. s.). Intense burning heat, beginning in the nape of neck and extending down spine, with a contractive stiffness, agg. by stretching. Rheumatism of the shoulder and arm, pains extend to fingers, agg. by motion (Right. Sang, Left – Ferr.). Lumbar vertebrae painful and sensitive to touch. Pain in sacrum, coccy and back of hips, running around and down the limbs. Pain in legs, from hips to knees; only when walking. Heaviness of legs, legs feel as if made up of lead walking very difficult legs are so heavy, legs give away. Lower limbs ache all night, preventing sleep. Intensely restless and fidgety of legs and feet. (Zinc.). Terrible suffering in legs and arms during an electrical storm. Aching in legs with inability to keep them still in bed, agg. when giving up control of himself, when relaxing, in trying to sleep. Coldness of legs and feet, of hands and forearms. Drawing, contracting sensation in hamstrings and ankles, cramps in calves and soles. (Cap.). Ankles easily turn when walking (Carb. an., Led.). Almost entire loss of power in legs and arms, exhausted by slightest effort. Painful stiffness of every joint in body. Deformity of finger joints; large, puffy knuckles; swelling and painful stiffness of ankles; great tenderness of heels and balls of feet; puffy wellings of all jointss like wind (balls)

Patient 1. Gastrointestinal tract : There is intense craving for liquor, which earlier she hated. The patient also desires salt, sweets, ice, acids, orange, green fruits. There is also ravenous hunger immediately after eating. Intense thirst; the patient even dreams of drinking 2. Stool : His stool is ball like. He feels constrictive feeling in the anus. He can pass stool only by leaning backward. There is sharp needle like pain in the rectum so as to cause crying. His stool is tenacious, clay like, sluggish with oozing of foetid moisture. 3. Throat : The patient is troubled with hoarseness especially while reading. There is choking caused by a weakness or spasm of the epiglottis. Her larynx gets constricted so that no air could either escape or enter. Relief obtained by lying on the face and protruding the tongue. There is a sensation as if cold has settled in throat with distressing pain and soreness. Throat is filled with thick, gray, blood stained mucus. The patient cannot swallow either liquid or solid. Throat complaints are aggravated from heat and when thinking of his ailments. 4. Cough : During cough there is a sensation of tickling under the sternum. It is incessant, dry and harassing. It is aggravated at night and when the patient is about to fall asleep. It is worse on lying down and it is better by lying on the stomach. There is constriction in throat. Cough is dry, continuous, deep, hollow, like coughing in a barrel, very difficult to raise sputa. If at all raised, it is small, frothy, green, in bitter balls and viscid. 5. Asthma : The patient suffers from asthma due to suppressed gonorrhoea. There is great dyspnoea, suffocation and choking caused by spasm and weakness of epiglottis. His larynx is completely stopped so that neo air can enter, Can inspire well, but can not expire. 6. Extremites : There is intense burning in feet and hands. The patient wants them uncovered and fanned. There is intense restlessness and fidgety feeling in legs, and feet. Pain in legs from hip to knee, only when walking, ache during night. During a thurderstorm, there is terrible burning and aching in legs and arms. 7. Rheumatism : Patient suffers from rheumatic pain dur to suppressed gonorrhoea. There is a sensation of constriction and tight feeling all over the body, sore feeling as if bruised. Both the arms and shoulders of the patient are affected. Painful stiffness of every joint in the body. Along with this there is deformity of finger joints, knuckles are large and puffy, swelling and painful stiffness of ankle. 8. Female genital organs : There is ulceration on the neck of the uterus. The menses are very dark and the stains are difficult to wash out. It is attended with intense colic causing drawing up of the knees. The pain is terrible with bearing down and labour like in nature. Menses are sometime followed by neuralgic pain in the head. Breast and nipples are very painful, sensitive and sore to touch. Her breasts especially the nipples are icy cold to touch during menses while rest of the body is warm.
